COVID-19
Researchers present an overview of the current state of knowledge on the SARS-CoV-2 and COVID-19 pandemic. In addition to an overview of the epidemiological, clinical, and radiological features of SARS-CoV-2, researchers also summarize possible therapeutic options currently under investigation and the future outlook for the disease. Whereas the trials on SARS-CoV-2 genome-based specific vaccines and therapeutic antibodies are currently being tested, this solution is more long-term, as they require thorough testing of their safety. On the other hand, the repurposing of the existing therapeutic agents previously designed for other virus infections and pathologies happens to be the only practical approach as a rapid response measure to the emergent pandemic. The current pandemic emergency will be a trigger for more systematic drug repurposing design approaches based on big data analysis. Further on, regression analytical review is presented on the virological and evolutionary history of SARS-CoV viruses, indicating to the autoimmune pathogen.

A Journey to Hear: The Evolution of Cochlear Implants
Cochlear implants (CIs), a revolutionary breakthrough in auditory technology, have profoundly impacted the lives of individuals with severe hearing impairment. Surgically implanted behind the ear and within the delicate cochlea, these devices represent a direct pathway to restoring the sense of hearing. Implanting hope alongside innovation, their captivating history unfolds through pivotal dates and transformative milestones. From the first human implantation by Drs. William House and John Doyle in 1961 to FDA approval in 1984, each step in their evolution mirrors a triumph of human ingenuity. The 1990s witnessed significant miniaturization, enhancing accessibility, while the 21st century brought about improvements in speech processing and electrode technology. These strides have elevated CIs beyond functional devices to life-changing instruments, enriching both auditory experiences and communication skills. This entry delves into the captivating history of CIs, spotlighting key dates that paint a vivid picture of challenges overcome and remarkable progress achieved. It explores the people and moments that defined their development, ultimately shaping these implants into indispensable tools that continually redefine the landscape of hearing assistance.

Berberine in Diabetes and Related Complications Treatment
Berberine (BBR) is an isoquinoline alkaloid that can be extracted from herbs such as Coptis, Phellodendron, and Berberis. BBR has been widely used as a folk medicine to treat various disorders. It is a multi-target drug with multiple mechanisms. Studies have shown that it has antioxidant and anti-inflammatory properties and can also adjust intestinal microbial flora.

Pharmacomicrobiomics and Drug–Infection Interactions
Microorganisms have a close relationship with humans, whether it is commensal, symbiotic, or pathogenic. It has been documented that microorganisms may influence the response to drug therapy. Pharmacomicrobiomics is an emerging field that focuses on the study of how variations in the microbiome affect the disposition, action, and toxicity of drugs. Two additional sciences have been added to complement pharmacomicrobiomics, namely toxicomicrobiomics, which explores how the microbiome influences drug metabolism and toxicity, and pharmacoecology, which refers to modifications in the microbiome as a result of drug administration. Additionally, the concept of "drug-infection interaction" is included to describe the influence of pathogenic microorganisms on drug response. This entry analyzes in detail each of these concepts.

Drug Therapies for Diabetes
The treatment of type 2 diabetes (T2D) necessitates a multifaceted approach that combines behavioral and pharmacological interventions to mitigate complications and sustain a high quality of life. Treatment encompasses the management of glucose levels, weight, cardiovascular risk factors, comorbidities, and associated complications through medication and lifestyle adjustments. Metformin, a standard in diabetes management, continues to serve as the primary, first-line oral treatment across all age groups due to its efficacy, versatility in combination therapy, and cost-effectiveness. Glucagon-like peptide-1 receptor agonists (GLP-1 RA) offer notable benefits for HbA1c and weight reduction, with significant cardiovascular benefits. Sodium-glucose cotransporter inhibitors (SGLT-2i) lower glucose levels independently of insulin while conferring notable benefits for cardiovascular, renal, and heart-failure outcomes. Combined therapies emphasizing early and sustained glycemic control are promising options for diabetes management. As insulin therapy remains pivotal, metformin and non-insulin agents such as GLP-1 RA and SGLT-2i offer compelling options. Notably, exciting novel treatments like the dual GLP-1/ glucose-dependent insulinotropic polypeptide (GIP) agonist show promise for substantially reducing glycated hemoglobin and body weight.

The Bioavailability of Drugs
Drug bioavailability is a crucial aspect of pharmacology, affecting the effectiveness of drug therapy. Understanding how drugs are absorbed, distributed, metabolized, and eliminated in patients’ bodies is essential to ensure proper and safe treatment. In addition to biochemical activity, bioavailability also plays a critical role in achieving the desired therapeutic effects. This may seem obvious, but it is worth noting that a drug can only produce the expected effect if the proper level of concentration can be achieved at the desired point in a patient’s body. Given the differences between patients, drug dosages, and administration forms, understanding and controlling bioavailability has become a priority in pharmacology.

Proteolytic Activation of the Epithelial Sodium Channel
Epithelial sodium channel (ENaC) are integral to maintaining salt and water homeostasis in various biological tissues, including the kidney, lung, and colon. They enable the selective reabsorption of sodium ions, which is a process critical for controlling blood pressure, electrolyte balance, and overall fluid volume. ENaC activity is finely controlled through proteolytic activation, a process wherein specific enzymes, or proteases, cleave ENaC subunits, resulting in channel activation and increased sodium reabsorption. This regulatory mechanism plays a pivotal role in adapting sodium transport to different physiological conditions.

Phase II Drug-Metabolizing Enzyme and Drug Transporter Expression
Sparse and conflicting evidence exists regarding the localization, expression, and regulation of phase II drug-metabolizing enzymes and drug transporters across gestational stages. To resolve the uncertainties and assumptions in current knowledge, additional pharmacokinetic (PK) data and clinical pharmacology research are required to understand drug metabolism and transport in the pregnant woman and in the placenta.

Development of Agents with Biological Activities by Umbelliferone
Umbelliferone (UMB), known as 7-hydroxycoumarin, hydrangine, or skimmetine, is a naturally occurring coumarin in the plant kingdom, mainly from the Umbelliferae family that possesses a wide variety of pharmacological properties. In addition, the use of nanoparticles containing umbelliferone may improve anti-inflammatory or anticancer therapy. Also, its derivatives are endowed with great potential for therapeutic applications due to their broad spectrum of biological activities such as anti-inflammatory, antioxidant, neuroprotective, antipsychotic, antiepileptic, antidiabetic, antimicrobial, antiviral, and antiproliferative effects.
