Transoral Robotic Surgery: Applications in HNC
Minimally invasive surgery is a growing field in surgical oncology. After acquiring its first Food and Drug Administration approval in 2009 for T1–T2 malignancies of the oral cavity, oropharynx, and larynx, transoral robotic surgery (TORS) has gained popularity thanks to its wristed instruments and magnified three-dimensional view, enhancing surgical comfort in remote-access areas. Its indications are expanding in the treatment of head and neck cancer, i.e., resection of tumors of the larynx, hypopharynx, or parapharyngeal space. Head and neck cancer (HNC) incidence has increased, ranking eighth among the most common cancers worldwide in 2020 with approximately 900,000 cases yearly. Following a similar trend, head and neck surgeons in Australia and New Zealand have a TORS adoption rate of 43.6%. The main indications for TORS in all these countries are lateral oropharyngectomy, base-of-tongue mucosectomy, and surgery for obstructive sleep apnea.

Transmission of Onychomycosis and Dermatophytosis between Household Members
Onychomycosis is a common fungal infection of the nail, caused by dermatophytes, non-dermatophytes, and yeasts. Predisposing factors include older age, trauma, diabetes, immunosuppression, and previous history of nail psoriasis or tinea pedis. Shared household equipment, including footwear, bedding, and nail tools, may transmit dermatophytes. The persistence of dermatophytes on household cleaning supplies, linen, and pets may serve as lasting sources of infection. Further investigation of the specific mechanisms behind household spread is needed to break the cycle of transmission, reducing the physical and social impacts of onychomycosis.

Transmembrane Protease Serine 2 in Prostate Cancer
The abundant expression of TMPRSS2 is evident in normal prostatic tissue. Different endogenous substrates of TMRPSS2 have been identified, including the epithelial sodium channel (ENaC) the protease-activated receptor-2 (PAR-2), as well as other serine protease-like zymogens, such as kallikrein-2 (KLK2), suggesting its potential contribution to prostate homeostasis and its plausible involvement in male fertility. The TMPRSS2:ERG fusion is considered relevant to prostate cancer, but its association with the development and progression as well as its clinical significance have not been fully elucidated.

Translational Stroke
The approach to reperfusion therapies in stroke patients is rapidly evolving, but there is still no explanation why a substantial proportion of patients have a poor clinical prognosis despite successful flow restoration. This issue of futile recanalization is explained here by three clinical cases which, despite complete recanalization, have very different outcomes. Preclinical research is particularly suited to characterize the highly dynamic changes in acute ischemic stroke and to identify potential treatment targets useful for clinical translation. This entry surveys the efforts taken so far to achieve mouse models capable to investigate of investigating the neurovascular underpinnings of futile recanalization. We highlight the translational potential of targeting tissue reperfusion in fully recanalized mouse models and of investigating the underlying pathophysiological mechanisms from subcellular to tissue scale. We suggest that stroke preclinical research should increasingly drive forward a continuous and circular dialogue with clinical research. When the preclinical and the clinical stroke research are consistent, translational success will follow.

Transjugular Intrahepatic Portosystemic Shunt in Hepatocellular Carcinoma Patients
Liver cancer is very frequent, and hepatocellular carcinoma (HCC) accounts for the majority of liver cancer cases. Its growing incidence has been greatly affected by the increasing prevalence of metabolic-associated fatty liver disease (MAFLD). The latter is a new epidemic in the era. In fact, HCC is often generated from noncirrhotic liver and its treatment benefits from surgical and nonsurgical approaches, potentially bridged by transjugular intrahepatic portosystemic shunt (TIPS) use. TIPS use is an effective treatment for portal hypertension complications, but its application in patients with HCC and clinically significant portal hypertension (CSPH) remains controversial due to concerns about tumor rupture, dissemination, and increased toxicity.
