Organic Agriculture affects Soil, Crop and Human Health
The underlying philosophy/hypothesis of organic farming—“healthy soils generate healthy crops which lead to healthy livestock and humans”—was first formulated by Sir Albert Howard and other organic farming pioneers. Organic crop production practices could increase the nutritional quality of food crops, the evidence now available suggests that there are nutritionally relevant composition differences between organic and conventional crops and that, overall, organic plant-foods contain higher concentrations of nutritionally desirable (phenolics, other antioxidants and/or mineral micronutrients) and lower concentrations of nutritionally undesirable chemicals (pesticide residues, cadmium, and/or Fusarium mycotoxins). However, agricultural intensification practices have reduced food quality and safety, the available evidence indicates that many “innovations” (monoculture/shorter rotations, mineral N and P fertilizers, pesticides, and short-straw cereal varieties) introduced as part of the agricultural intensification (or “green revolution”) have had negative effects on both crop health and the nutritional quality and/or safety of crops.

Macromolecule entanglements are common in polymers. The chains of macromolecules with carbon skeletons are flexible. The isolated chain easily takes the shape of a coil. When a macromolecule is surrounded by other macromolecules, its coils interpenetrate, and entanglements arise between these macromolecules. The condition of their occurrence is that their molecular weight exceeds a certain limit. The entanglements may be topological (these are common) or cohesive. Appropriate pain care should be regarded as a right and effectively guaranteed to people with chronic pain (CP). Additionally, the World Health Organization (WHO) has recognized the importance of addressing CP as a global public health concern and has recommended that pain management be integrated into primary healthcare systems. Many individuals with CP continue to experience barriers to accessing appropriate and effective pain care, including a lack of education and training among healthcare providers, limited availability of specialized pain clinics, and misconceptions about the use of opioid medications for pain management. Furthermore, CP patients often encounter social stigmatization and discrimination, which can further complicate their access to care. There is a need for ongoing efforts to improve the recognition, assessment, and management of chronic pain, and to ensure that individuals with CP have access to appropriate and effective pain care that addresses their physical, psychological, and social needs. Law 38, enacted in Italy in 2010, establishes the citizen’s right not to suffer.

HIV-1 Latency and Viral Reservoirs
Eradication of latent human immunodeficiency virus (HIV) infection is a global health challenge. Reactivation of HIV latency and killing of virus-infected cells, the so-called “kick and kill” or “shock and kill” approaches, are a popular strategy for HIV cure. While antiretroviral therapy (ART) halts HIV replication by targeting multiple steps in the HIV life cycle, including viral entry, integration, replication, and production, it cannot get rid of the occult provirus incorporated into the host-cell genome. These latent proviruses are replication-competent and can rebound in cases of ART interruption or cessation. In general, a very small population of cells harbor provirus, serve as reservoirs in ART-controlled HIV subjects, and are capable of expressing little to no HIV RNA or proteins. Beyond the canonical resting memory CD4+ T cells, HIV reservoirs also exist within tissue macrophages, myeloid cells, brain microglial cells, gut epithelial cells, and hematopoietic stem cells (HSCs). Despite a lack of active viral production, latently HIV-infected subjects continue to exhibit aberrant cellular signaling and metabolic dysfunction, leading to minor to major cellular and systemic complications or comorbidities. These include genomic DNA damage; telomere attrition; mitochondrial dysfunction; premature aging; and lymphocytic, cardiac, renal, hepatic, or pulmonary dysfunctions. Therefore, the arcane machineries involved in HIV latency and its reversal warrant further studies to identify the cryptic mechanisms of HIV reservoir formation and clearance.

Supplements for Smoking-Related Lung Diseases
Supplements for smoking-related lung diseases are considered as nonfood products and thought to improve health. Multivitamins and antioxidants are the most commonly dietary supplements used by cancer and asthma patients. There are currently no clear regulatory guidelines that include dietary supplements and their effect on lung cancer and asthma patients, particularly in smokers. Several countries have taken steps to overcome challenges in regulating dietary supplements in the marketplace. These challenges include inadequate assurance of safety/efficacy, inaccuracy of product labeling, misleading health claims, and lack of analytical techniques for dietary supplements. There is a need to establish standards and regulation of dietary supplement use in patients with lung cancer and asthma. The aim of this entry is to expand knowledge on dietary supplements use and smoking-related lung diseases (lung cancer and asthma).

The rise in musculoskeletal disorders has prompted medical experts to devise novel effective alternatives to treat complicated orthopedic conditions. The ever-expanding field of regenerative medicine has allowed researchers to appreciate the therapeutic value of bone marrow-derived biological products, such as the bone marrow aspirate (BMA) clot, a potent orthobiologic which has often been dismissed and regarded as a technical complication. Numerous in vitro and in vivo studies have contributed to the expansion of medical knowledge, revealing optimistic results concerning the application of autologous bone marrow towards various impactful disorders. The bone marrow accommodates a diverse family of cell populations and a rich secretome; therefore, autologous BMA-derived products such as the “BMA Matrix”, may represent a safe and viable approach, able to reduce the costs and some drawbacks linked to the expansion of bone marrow. BMA eliminates many hurdles associated with its preparation, especially in regards to regulatory compliance. The BMA Matrix represents a suitable alternative, indicated for the enhancement of tissue repair mechanisms by modulating inflammation and acting as a natural biological scaffold as well as a reservoir of cytokines and growth factors that support cell activity. Although promising, more clinical studies are warranted in order to further clarify the efficacy of this strategy.

N-acetylcysteine (NAC), a plant antioxidant naturally found in onion, is a precursor to glutathione. It has been used as a drug since the 1960s and is listed on the World Health Organization (WHO) Model List of Essential Medicines as an antidote in poisonings. There are numerous other uses or proposed uses in medicine that are still in preclinical and clinical investigations. NAC is also used in food supplements and cosmetics. Despite its abundant use, there are projections that the NAC global market will grow in the next five years; therefore, the purpose of this work is to provide a balanced view of further uses of NAC as a dietary supplement. Although NAC is considered a safe substance, the results among clinical trials are sometimes controversial or incomplete, like for many other antioxidants. More clinical trials are underway that will improve our understanding of NAC applicability.
