Mediterranean Diet and Potential Drug-Food Interactions
There is a growing interest among people in western countries for adoption of healthier lifestyle habits and diet behaviors with one of the most known ones to be Mediterranean diet (Med-D). Med-D is linked with daily consumption of food products such as vegetables, fruits, whole grains, seafood, beans, nuts, olive oil, low-fat food derivatives and limited consumption of meat or full fat food products. Med-D is well-known to promote well-being and lower the risk of chronic conditions such as cardiovascular diseases, diabetes, and metabolic syndrome. On the other hand, bioactive constituents in foods may interfere with drugs’ pharmacological mechanisms, modulating the clinical outcome leading to drug-food interactions (DFIs). This text discusses current evidence for food products that are included within the Med-D and available scientific data suggest a potential contribution in DFIs with impact on therapeutic outcome. Proper patient education and consultation from healthcare providers is important to avoid any conflicts and side effects due to clinically significant DFIs.

Antimicrobial Resistance with Infectious Diseases during Hajj
Hajj pilgrimage is a large mass gathering global event that may facilitate the spread and emergence of various infectious diseases as well as antimicrobial resistance (AMR) in a local and global scenario. Planning and preparing for these public health issues is a challenging and complex process for the Kingdom of Saudi Arabia (KSA) health authorities. Despite multiple efforts for the prevention and treatment of infectious diseases through longtime funding in education and medical care, the prevalence of infectious disease is still high among Hajj pilgrims. The commonly observed infectious diseases during Hajj include respiratory tract infections (influenza and pneumonia), urinary tract infections and skin infections that may necessitate the use of antimicrobials. Beta-lactams are used as a first-line treatment for hospital acquired infections as well as community acquired infections due to their broad-spectrum activity. However, most of the bacterial isolates such as Staphylococcus spp., Pseudomonas spp. and E. coli are resistant to beta-lactams. Irrational use of antimicrobials, lack of infection prevention practices and suboptimal healthcare access further exacerbate the risk of spreading AMR among Hajj pilgrims.

Chromosome Conformation Signatures
Epigenetic mechanisms mediate the integration of genetic and environmental factors that are responsible for changes in phenotypes. The organisation of the human genome within three-dimensional space (the 3D genome) is a dynamic epigenetic regulator of phenotypic expression. Mediated through the changing spatial proximity of genomic regions relative to one another in 3D space, ‘chromosome conformations’ have emerged in the past several years as a novel class of molecular switches that regulate cellular and physiological processes. Technological advances in the detection of chromosome conformations have spawned a new class of biomarker - the chromosome conformation signature (CCS) - that identifies chromosomal interactions across multiple genomic loci as a collective marker of distinct epigenomic states. The use of CCSs in basic and clinical research has shown recent applications in identifying disease states, subtyping disease states, and prospectively stratifying individuals according to their likely response to medical intervention.

Endothelial to mesenchymal transition
Lung diseases, such as pulmonary hypertension and pulmonary fibrosis, are life-threatening diseases and have common features of vascular remodeling. During progression, extracellular matrix protein deposition and dysregulation of proteolytic enzymes occurs, which results in vascular stiffness and dysfunction. Although vasodilators or anti-fibrotic therapy have been mainly used as therapy owing to these characteristics, their effectiveness does not meet expectations. Therefore, a better understanding of the etiology and new therapeutic approaches are needed. Endothelial cells (ECs) line the inner walls of blood vessels and maintain vascular homeostasis by protecting vascular cells from pathological stimuli. Chronic stimulation of ECs by various factors, including pro-inflammatory cytokines and hypoxia, leads to ECs undergoing an imbalance of endothelial homeostasis, which results in endothelial dysfunction and is closely associated with vascular diseases.

Toxoplasma gondii (T. gondii) is a prevalent protozoan parasite of medical and veterinary significance. It is the etiologic agent of toxoplasmosis, a neglected disease in which incidence and symptoms differ between patients and regions. In immunocompetent patients, toxoplasmosis manifests as acute and chronic forms. Acute toxoplasmosis presents as mild or asymptomatic disease that evolves, under the host immune response, into a persistent chronic disease in healthy individuals. Chronic toxoplasmosis establishes as latent tissue cysts in the brain and skeletal muscles. Valorization of Grape by-products
The emergence of antibiotic-resistance in bacteria has limited the ability to treat bacterial infections, besides increasing their morbidity and mortality at the global scale. The need for alternative solutions to deal with this problem is urgent and has brought about a renewed interest in natural products as sources of potential antimicrobials. The wine industry is responsible for the production of vast amounts of waste and by-products, with associated environmental problems. These residues are rich in bioactive secondary metabolites, especially phenolic compounds. Some phenolics are bacteriostatic/bactericidal against several pathogenic bacteria and may have a synergistic action towards antibiotics, mitigating or reverting bacterial resistance to these drugs. Complex phenolic mixtures, such as those present in winemaking residues (pomace, skins, stalks, leaves, and especially seeds), are even more effective as antimicrobials and could be used in combined therapy, thereby contributing to management of the antibiotic resistance crisis.

Gluten Conformation at Different Temperatures and Additive Treatments
The effect of temperature (25, 45, and 65 °C) on the gluten secondary structure was investigated by using Fourier transform infrared (FTIR) spectroscopy and modulation of disulfide and hydrogen bonds contributions (100 ppm ascorbic acid (AA), 0.6% diacetyl tartaric acid ester of monoglycerides (DATEM), and 0.25 mM dithiothreitol (DTT)). The results showed that additives heated at 65 °C altered most of the gluten matrix formation by changing structural secondary structures compared to the secondary structures of native gluten (control). The content of random coils, α-helices, and β-sheet of gluten increased, while the extent of β-turns and antiparallel β-sheets decreased, which led to the transformation to a more stable secondary conformation. In addition, the rheological properties (%creep strain) revealed that gluten deformation increased during the heating process with all of the additives. The chemometric method could quantitate an overall alteration of gluten polymerization and gluten matrix formation during heating with additive treatments.

A medical sign is an objective indication of a disease, injury, or abnormal physiological state that may be detected during the physical examination of a patient. These signs can be detectable by anyone, e.g. the temperature or blood pressure of the patient, skin that is redder than usual, or a bruise; others may have no meaning to the patient or may even go completely unnoticed. Medical signs assist a healthcare provider to reach an accurate diagnosis. A symptom is something, such as pain or dizziness, experienced by the patient. Signs and symptoms are not mutually exclusive. In other words, many symptoms such as feeling hot, are associated with a medical sign such as a thermometer reading showing an above average body temperature. Symptoms and signs are often nonspecific, but certain combinations can be suggestive of certain diagnoses, helping to narrow down what may be wrong. Examples of signs include elevated blood pressure, clubbing of the ends of fingers as a sign of lung disease, staggering gait, and arcus senilis of the eyes. In medicine, a sign is distinguished from an indication; a specific reason for using a treatment.
