Relationship between Gut Microbiota and Cardiovascular Disease
Cardiovascular disease (CVD) is a leading cause of death and morbidity worldwide. Identification of possible preventive strategies is of central interest to avoid CVD onset and progression. In this regard, intensive medical and scientific activity is currently employed in the management of traditional risk factors, but despite maximum medical therapy, there is still a residual risk of undetermined etiology. The gut microbiota is gaining increasing interest as one of the potentially modifiable factors that are involved in the pathogenesis of several diseases, including CVD. A multitude of microorganisms lives symbiotically with the human host; 10–100 trillion microbes among bacteria, viruses, fungi, and helminths are located in the gut. They serve a multitude of functions, which include maintenance of intestinal homeostasis and defence against external aggressive agents, modulation of the immune response, and production of metabolites.

Oxidative stress (OxS) is a physiologically significant alteration in redox status resulting from the overproduction of reactive species and or the reduction in antioxidant defenses. Oxidative stress (OxS) has emerged as a likely initiating factor in T2D. Antioxidant supplements may act to slow or prevent T2D by multiple mechanisms, i.e., (1) reducing mitochondrial oxidative stress, (2) preventing the damaging effects of lipid peroxidation, and (3) acting as essential cofactors for antioxidant enzymes.

Euryale ferox Salisb. (prickly water lily) is the only extent of the genus Euryale that has been widely distributed in China, India, Korea, and Japan. The seeds of E. ferox (EFS) have been categorized as superior food for 2000 years in China, based on their abundant nutrients including polysaccharides, polyphenols, sesquineolignans, tocopherols, cyclic dipeptides, glucosylsterols, cerebrosides, and triterpenoids. These constituents exert multiple pharmacological effects, such as antioxidant, hypoglycemic, cardioprotective, antibacterial, anticancer, antidepression, and hepatoprotective properties.

Central Nervous System Diseases and the Probenecid
Probenecid is an old uricosuric agent used in clinics to treat gout and reduce the renal excretion of antibiotics. In recent years, probenecid has gained attention due to its ability to interact with membrane proteins such as TRPV2 channels, organic anion transporters, and pannexin 1 hemichannels, which suggests new potential therapeutic utilities in medicine. Some current functions of probenecid include their use as an adjuvant to increase the bioavailability of several drugs in the Central Nervous System (CNS). Numerous studies also suggest that this drug has important neuroprotective, antiepileptic, and anti-inflammatory properties, as evidenced by their effect against neurological and neurodegenerative diseases. In these studies, the use of probenecid as a Panx1 hemichannel blocker to reduce neuroinflammation is highlighted since neuroinflammation is a major trigger for diverse CNS alterations.

Genome mining has allowed for more targeted excavation of biosynthetic potential from natural sources that was previously overlooked. Researchers use bioinformatic algorithms to rapidly identify and predict antimicrobial candidates by studying the genome before even entering the lab.

Principles of pH-Responsive Drug Delivery
The paradigm of drug carriers’ usage to overcome the non-specific distribution of therapeutic agents in the body, including chemotherapeutic substances that exert severe toxic stress on healthy tissues, has been actively developed. One of the main pillars of this paradigm is the increased or even selective accumulation of drug delivery systems (DDSs) carrying therapeutic agents in tumor interstitium harnessing the differences between normal and cancer tissues properties. Thus, structural features of tumors, such as hypervascularization, vascular pathologies, and impaired functionality of lymphatic drainage, can be utilized to differentiate tumors from healthy tissues and selectively accumulate drug carriers. In particular, tumor-surrounding vessels are characterized by defects in the endothelial layer lining the blood vessel wall, represented by wide fenestrations (up to several microns) and other features that lead to an increase in the permeability of this barrier for small objects, making the effective extravasation of nanosized carriers from the bloodstream to tumor interstitium possible. Methods of selective therapy via the systemic administration of therapeutic agents based on increased permeability of the tumor vessels’ wall, known under the general name of the EPR effect, have become widespread and have inspired the creation of a large number of vehicles proposed for the delivery of chemotherapeutic agents. In summary, the EPR effect implies the extravasation of nanosized drug carriers through endothelial fenestra and their retention in the interstitial volume of the tumor due to dysfunctional lymphatic drainage.

Antibiotic stewardship (AS) programs have become a priority for health authorities to reduce the number of infections by super-resistant microorganisms. The need for these initiatives to minimize the inadequate use of antimicrobials is essential, and the election of the antibiotic in the emergency department usually impacts the choice of treatment if the patients need hospital admission, becoming an opportunity for antibiotic stewardship. In the pediatric population, broad-spectrum antibiotics are more likely to be overprescribed without any evidence-based management, and most of the publications have focused on the prescription of antibiotics in ambulatory settings.
