HIF-1α in IVD Treatment
The intervertebral disc (IVD) is a complex joint structure comprising three primary components—namely, nucleus pulposus (NP), annulus fibrosus (AF), and cartilaginous endplate (CEP). The IVD retrieves oxygen from the surrounding vertebral body through CEP by diffusion and likely generates ATP via anaerobic glycolysis. IVD degeneration is characterized by a cascade of cellular, compositional, structural changes. With advanced age, pronounced changes occur in the composition of the disc extracellular matrix (ECM). NP and AF cells in the IVD possess poor regenerative capacity compared with that of other tissues. Hypoxia-inducible factor (HIF) is a master transcription factor that initiates a coordinated cellular cascade in response to a low oxygen tension environment, including the regulation of numerous enzymes in response to hypoxia. HIF-1α is essential for NP development and homeostasis and is involved in various processes of IVD degeneration process, promotes ECM in NP, maintains the metabolic activities of NP, and regulates dystrophic mineralization of NP, as well as angiogenesis, autophagy, and apoptosis during IVD degeneration. HIF-1α may, therefore, represent a diagnostic tool for early IVD degeneration and a therapeutic target for inhibiting IVD degeneration

Plasma Proteins
Blood contains various plasma proteins and cells to which endogenous and exogenous molecules can bind together to be transported throughout the circulatory system. Among the different plasma proteins, the binding of small molecular weight drug molecules is mostly associated with serum albumin and alpha-1 acid glycoprotein and, to a lesser extent globulins and lipoproteins. Although linear binding applies for most clinically used drugs, some physiopathological factors and/or dietary-drug interactions may lead to alterations of the drug-protein binding, which in turn may result in clinically changes in the pharmacokinetics and pharmacodynamics of the drug. As polyphenols (flavonoids and non-flavonoids) are widely present in plant-derived feeds, beberages, herbal medicines, and dietary supplements, the knowledge of how they bind to plasma proteins can prevent/avoid such interactions.

Nitroglycerin (Medication)
Nitroglycerin, also known as glyceryl trinitrate (GTN), is a medication used for heart failure, high blood pressure, anal fissures, painful periods, and to treat and prevent chest pain caused by decreased blood flow to the heart (angina) or due to the recreational use of cocaine. This includes chest pain from a heart attack. It is taken by mouth, under the tongue, applied to the skin, or by injection into a vein. Common side effects include headache and low blood pressure. The low blood pressure can be severe. It is unclear if use in pregnancy is safe for the baby. It should not be used together with medications within the sildenafil (PDE5 inhibitor) family due to the risk of low blood pressure. Nitroglycerin is in the nitrate family of medications. While it is not entirely clear how it works, it is believed to function by dilating blood vessels. Nitroglycerin was written about as early as 1846 and came into medical use in 1878. It is on the World Health Organization's List of Essential Medicines. The drug nitroglycerin (GTN) is a dilute form of the same chemical used as the explosive, nitroglycerin. Dilution makes it non-explosive. In 2019, it was the 184th most commonly prescribed medication in the United States, with more than 3 million prescriptions.

Dual-Modular Stems for Primary Total Hip Arthroplasty
In primary total hip arthroplasty (THA), dual-modular stems were introduced to better restore hip stability, femoral offset, and leg length. This entry highlights the gathered knowledge about dual-modular stems and related complications in combinations with titanium (Ti) and cobalt-chrome (CoCr) exchangeable necks. The reasons for a modular neck failure are multifactorial. Some of the dual-modular stems are still on the market despite the fact th these designs have neither been proven for durability nor have shown any clinical benefits for the patients as compared to monolithic stems. Apart from very limited indications, orthopaedic surgeons should not use dual-modular stem designs for primary THA.
