Oxidative Stress in Pulmonary Hypertension
Pulmonary hypertension (PH) is a progressive disease characterized by elevated artery pressures and pulmonary vascular resistance. Underlying mechanisms comprise endothelial dysfunction, pulmonary artery remodeling and vasoconstriction. Several studies have shown evidence of the critical role of oxidative stress in PH pathophysiology. Alteration of redox homeostasis produces excessive generation of reactive oxygen species, inducing oxidative stress and the subsequent alteration of biological molecules. Exacerbations in oxidative stress production can lead to alterations in nitric oxide signaling pathways, contributing to the proliferation of pulmonary arterial endothelial cells and smooth muscle cells, inducing PH development.

Splanchnic vein thrombosis (SVT) includes portal (portal vein thrombosis, PVT), mesenteric (mesenteric vein thrombosis, MVT) and splenic vein thrombosis, and Budd-Chiari syndrome (BCS). SVT is generally classified as secondary to an identified risk factor or primitive, unprovoked, when causative factors cannot be identified. While much rarer than common venous thromboembolism (VTE), SVT is often challenging to clinicians for both the identification of the causal disorder and for its therapeutic management. The therapeutic approach of SVT is a clinical challenge and considers the manifestations and the site of thrombosis, the risk of SVT progression, recurrence and bleeding. Ischemia with nonobstructive coronary artery disease (INOCA) is increasingly recognized as a significant cause of angina, myocardial remodeling, and eventually heart failure (HF). Coronary microvascular dysfunction (CMD) is a major endotype of INOCA, and it is caused by structural and functional alterations of the coronary microcirculation. At the same time, atrial cardiomyopathy (ACM) defined by structural, functional, and electrical atrial remodeling has a major clinical impact due to its manifestations: atrial fibrillation (AF), atrial thrombosis, stroke, and HF symptoms. Both these pathologies share similar risk factors and have a high comorbidity burden. CMD causing INOCA and ACM frequently coexist.
