Congenital Afibrinogenemia and Hypofibrinogenemia
Congenital fibrinogen disorders are rare pathologies of the hemostasis, comprising quantitative and qualitative disorders. The clinical phenotype is highly heterogeneous, being associated with bleeding, thrombosis, or absence of symptoms. Afibrinogenemia and hypofibrinogenemia are the consequence of mutations in the homozygous, heterozygous, or compound heterozygous state in one of three genes encoding the fibrinogen chains, which can affect the synthesis, assembly, intracellular processing, stability, or secretion of fibrinogen. In addition to standard coagulation tests depending on the formation of fibrin, diagnostics also includes global coagulation assays, which are effective in monitoring the management of replacement therapy. Genetic testing is a key point for confirming the clinical diagnosis. The identification of the precise genetic mutations of congenital fibrinogen disorders is of value to permit early testing of other at risk persons and better understand the correlation between clinical phenotype and genotype.

Fibrinogen in Hypofibrinogenemia
Congenital fibrinogen disorders are diseases associated with a bleeding tendency; however, there are also reports of thrombotic events. Fibrinogen plays a role in the pathogenesis of thrombosis due to altered plasma concentrations or modifications to fibrinogen’s structural properties, which affect clot permeability, resistance to lysis, and its stiffness. Several distinct types of genetic change and pathogenetic mechanism have been described in patients with bleeding and a thrombotic phenotype, including mutations affecting synthesis or processing in three fibrinogen genes.

Sting-Related Deaths in Europe (1994–2016)
Bees and wasps provide an important service to ecosystems, contributing to the improvement of biodiversity while helping to maintain ecological balance. Although not frequent, hornet, wasp, and bee stings may be life-threatening. With each sting, Hymenopterans inject a small amount of venom that can cause reactions of varying intensity: (i) normal local reactions (NLR), (ii) large local reactions (LLR), (iii) systemic anaphylactic reactions (SAR), (iv) systemic toxic reactions (STR) and (v) unusual reactions (UR).  Hymenopteran stings have recently become a worldwide public health concern. However, this health problem can be underestimated despite the number of cases presented. With the expansion of non-native Hymenopteran species across Europe, allergists should be aware that their community’s exposures are continually changing.

Stability Modelling of mRNA Vaccine Quality
The vaccine distribution chains in several low- and middle-income countries are not adequate to facilitate the rapid delivery of high volumes of thermosensitive COVID-19 mRNA vaccines at the required low and ultra-low temperatures. COVID-19 mRNA vaccines are currently distributed along with temperature monitoring devices to track and identify deviations from predefined conditions throughout the distribution chain. These temperature readings can feed into computational models to quantify mRNA vaccine critical quality attributes (CQAs) and the remaining vaccine shelf life more accurately. Here, a kinetic modelling approach is proposed to quantify the stability-related CQAs and the remaining shelf life of mRNA vaccines. The CQA and shelf-life values can be computed based on the conditions under which the vaccines have been distributed from the manufacturing facilities via the distribution network to the vaccination centres.

In China, the population aged 60 and higher was 253.88 million in 2019, accounting for 18.1% of China’s total population (National Bureau of Statistics 2020), which far exceeds the international standards of aging. Furthermore, the proportion of the older population is still rising.
